Instructions for New Parents: **Your student must be registered with the district and be assigned a student number and student e-mail address before you may register them on FinalForms.** Once on the Zeeland FinalForms website, parents new to FinalForms should click on * New Account, as seen below, and enter the required information. A confirmation email will be sent to the e-mail address you provide. Click on the link in the email to activate your Parent Account and create a password. Once logged into your Parent Account, click on the box, as seen below, to switch to the 2023-2024 registration year. DO NOT complete any information without switching to the 2023-2024 year first. This box will be visible until July 10, 2023, after which the 2023-2024 registration year will be the default. From here, NEW parents should complete the following steps:
  1. +Register Student - Fill out basic information about your student. You will need your student's ZPS e-mail address, ending in @zpsstudents.org, in order to register them on FinalForms. Once you complete this step on FinalForms, your student will be registered as a student-athlete for Zeeland on FinalForms and you will always be able to view them under My Students. Complete the remaining steps for your student to be eligible to participate.
  2. +Add Sports - For each student-athlete, parents must select all sports that their child is considering trying out for or participating in. Student-athletes will not be allowed to try out/participate until they are registered under that sport on FinalForms. Parents must select at least one sport for each student-athlete. This must be completed prior to completing or updating any forms for that athlete, otherwise certain required forms may not be available to complete.
  3. Update Forms - Carefully read through all forms and complete all required fields. Some forms may contain links to important documents. Please read through all linked materials before signing each form. A parent signature is required on all forms. Student signatures are also required on select forms. Information for creating a Student Account can be found below.
  4. $ Pay Fee - FinalForms requires a registration fee for each student-athlete. This fee is required only once per year when parents register their student on FinalForms. If this fee is not paid before the student-athlete's first contest, including scrimmages, the athlete will not be eligible to participate until the fee has been paid. The FinalForms fee can be paid under your FinalForms Parent Account or in your school's respective Athletic Office.
  5. Pre-participation Physical Exam (PPE) - All student-athletes are required to obtain a pre-participation physical exam performed by a medical provider (i.e., MD, DO, NP, PA) on or after April 15, 2023 for the 2023-2024 academic year.
    1. To print the PPE form from FinalForms before your physical exam: click on +PPE Medical next to their student's name on the Parent Account page, as seen below. This form should be taken with the student to their physical exam performed at their primary care office.






  1. The completed form can be uploaded directly into your student’s FinalForms account. It will be reviewed by the Athletic Office and turned “green” if it is acceptable. If you are unable to upload, you may turn in completed physical forms to each student's respective Athletic Office prior to the first day of tryouts/practice for the student to be eligible.
  1. Baseline Concussion Testing Instructions
  2. Please READ and understand all instructions for each section before continuing, failure to do so may result in an invalid test meaning you have to take it again. Go to the Concussion Vital Signs website and click on Athlete Testing on the right-hand side Enter the information exactly as it says in the picture below; all athletes will use this username and password to access our school testing account. User Name: athlete321 Password: Zeeland22*

    Enter your Student # (eight digits)

    Click Take the Test

  9. Please do the following before starting your test:
  10. Make sure you have a device (a computer, laptop or Chromebook) with an external mouse or trackpad (a tablet, iPad or phone will not work)
  11. Find a quiet and distraction free environment, no phones or other devices
  12. Sit comfortably at a table or desk with a flat, hard surface
  13. Close all other programs and browser tabs
  14. Disable popup blockers during the test

  15. Be sure to use your FULL name (first and last), no nicknames, when entering your name
  16. You must complete the test in a single attempt of less than 45 minutes
  17. FinalForms will be updated weekly by your athletic trainer
  18. FAQ’s How do I check the status of my child's baseline concussion test?
  19. Login into your parent account on FinalForms .
  20. Click on Profile under your student-athlete's name
  21. Scroll down to Concussion Test History
    • A current test will appear as a green box with a number and clock symbol. This number indicates the number of days until this test expires. You may also hover over the number with your cursor and it will display the exact date the test will expire.
    • Any tests that appear with a red or yellow number means the test is either currently expired or will soon expire.
    • If your 2023-2024 high school student-athlete does not have a test with a green icon, they will need to complete a baseline concussion testing session.
  22. What is the baseline concussion test for? The baseline concussion test is an individualized analysis of a student-athlete’s neurocognitive brain function in a healthy state. In the event that a student-athlete sustains a head injury, this analysis will be used as a normal, baseline value to compare to the post-injury test value to help determine which, if any, neurocognitive functions are impaired by the injury. The concussion vital signs test is not considered a diagnostic tool, but is used in the evaluation and management process for concussions. Why is the baseline test only valid for two years? Because brains in high school develop at such a quick rate it is recommended that athletes be tested every two years to ensure that the baseline test reflects where the brain is currently functioning.

Instructions for Returning Parents: Parents returning to FinalForms from a previous year should use their same login to access their Parent Account. From your Parent Account click on the box, as seen below, to switch to the 2023-2024 registration year. DO NOT update any information without switching to the 2023-2024 year first. This box will be visible until July 10, 2022, after which the 2023-2024 registration year will be the default. From here, returning parents should complete the following steps:
  1. +Add Student - Any students who will be in grades 6-12 during the 2023-2024 academic year and intend to play a school sport should appear under My Students on your Parent Account. If a student does not appear, parents will need to add them and complete the corresponding student information and forms. Remember that you will need your student's ZPS e-mail, ending in @zpsstudents.org, to register them.

  1. Sign Forms - When you log in to your Student Account, you will be navigated directly to the forms you are required to sign as a student. Read carefully through each form, including any linked materials, such as the Athletic Code of Conduct and Acknowledgement of Risk, prior to signing the bottom of each form.
  2. Physical Exam - All student-athletes are required to obtain a physical exam performed by a medical provider (i.e., MD, DO, NP, PA) on or after April 15, 2023 for the 2023-2024 academic year. Once all forms have been completed and signed by the parent and student. Upload your completed form to your FinalForms account or turn in completed physical form to your respective Athletic Office prior to the first day of tryouts/practice in order to be eligible.
  3. Baseline Testing - All high school student-athletes are required to have a current baseline test prior to participating in a school-sponsored sport. Current baseline tests are those taken within two academic years (typically completed before a student's 9th and 11th grade years).

Instructions for Returning Students: All returning students will need to log into their previously created Student Account on the Zeeland Final Forms website by clicking the Student Login button as seen below using your ZPS student email address. Once logged into your student account, complete the following steps:
  1. Sign Forms - When you log in to your Student Account, you will be navigated directly to the forms you are required to sign as a student. Read carefully through each form including any linked materials, such as the Athletic Code of Conduct and Acknowledgement of Risk, prior to signing the bottom of each form.
  2.
